A physician visit is going and seeing a doctor at his office where as a consulting physician would be in a hospital situation where a doctor comes to you and works on you.

It is something similar to the difference between “Outpatient” and “Inpatient”. ‘Inpatient’ is while admitted to a hospital and ‘Outpatient’ could be at a hospital for some test or at a doctor’s office or specialty office while not being a hospitalized patient.

02. Look back period for pre-existing conditions

Thank you for your prompt response on the plans suitable for pre existing conditions. I am interested in buying the Atlas plan but I have some questions.

It is clear the meaning of "acute onset of a pre existing condition". But I have a doubt about pre existing condition.

For example: My husband has an aorta surgery on 2012. Is this a pre-existing condition? Considering it was on 2012 on in some plans you mention that a pre existing condition has to refer an event happened two year or three year ago. Could you please, explain a little more about this issue?

If the look back period for pre existing condition is 2 years and the event has occurred before that then it is not a pre existing condition.

But if the person is still on medication for this condition then it is still considered a pre existing condition. However, the company Worldtrips who is the insurer of Atlas plan exclude coverage for chronic illnesses. Heart condition are generally chronic and does not go away. In your situation it will be a pre existing condition.

03. Finding PPO Network

Regarding paying the claims it is dependent on the provider. Sometimes they might directly call the Insurance Company or else you will have to make the payment first and get it reimbursed from the company by filling their claim form. Claim forms are available here: Claims

04. Query regarding renewal of policy for travelers aged 69 and 70 years

questionHello,

I currently have Atlas International Policy E91107720 which is valid to 30 Jan 2015. At that time I will still be 69 years old and I would like to renew the policy for one year. On 03 Jul 2015 I will turn 70.

My question is: if I renew the policy while I am still 69 and turn 70 during the policy period, will the amount of coverage (currently $500,000) remain the same when I turn 70 in the middle of what would be the then existing policy period? Please advise!

Does this policy provide sports coverage for martial arts like Aikedo? Martial Arts is excluded in Atlas policy. Please see the Atlas sports exclusion in the brochure: Brochure

Can you please elaborate on precertification process related to hospitalization?

The following expenses must always be Pre-certified:

  Inpatient care

  Any Surgery or Surgical Procedure

  Care in an Extended Care Facility

  Home Nursing Care

  Durable Medical Equipment

  Artificial limbs

  Computerized Tomography (CAT Scan)

  Magnetic Resonance Imaging (MRI)

To comply with the Pre-certification requirements, the Member must:

1. Contact the Plan Administrator at the telephone number contained in the Member’s Certificate as soon as possible before the expense is to be incurred; and

2. Comply with the instructions of the Plan Administrator and submit any information or documents they require; and

3. Notify all Physicians, Hospitals and other providers that this insurance contains Pre-certification requirements and ask them to fully cooperate with the Plan Administrator.

If the Member complies with the Pre-certification requirements, and the expenses are Pre-certified, Underwriters will pay Eligible Medical Expenses subject to all terms, conditions, provisions and exclusions herein. If the Member does not comply with the Pre-certification requirements or if the expenses are not Pre-certified:

1. Eligible Medical Expenses will be reduced by 50%; and

2. The Deductible will be subtracted from the remaining amount; and

3. The Coinsurance will be applied

Does claim forms need to be submitted every time and how can it be paid?

A claims form must be submitted for each diagnosis. For example if a member breaks an arm, they submit a claims form and accident questionnaire with bills / receipts. If they have follow up treatment for that same injury, they do not need to complete and submit a new claims form. If we can arrange direct billing with the facility, we pay the facility directly. If the member has to pay upfront for the services, we will provide a reimbursement directly to the member for eligible expenses.

I am assuming that this policy covers doctor visits but not routine check ups?

08. Problem with selection of destination as USA, from Spain visiting New York and Boston

When you begin the application, it asks if the US or US territories will be your destination country. You must answer YES to this question. Also, if you have an issue, close out your browser and try again. The site is a little quirky and sometimes just doesn't work even if you did do that.

09. Travel Health Insurance Plan for J1 Visa holders from Africa

Our organization in the United States will be potentially hosting a group of exchange students (35-43 participants, ages 15-18) from Africa next year for a (3-4) week period.

The State Department requires the following insurance for the participants of this exchange:

Exchange program regulations require that all J-l visa holders carry health and accident insurance. At a minimum, insurance must provide the following benefits:

1. medical coverage of at least $50,000 per person per accident or illness;

2. repatriation of remains in the amount of $7,500; and

3. medical evacuation benefits of at least $10,000.

In addition, a qualified insurance program shall not have a deductible that exceeds $500 and it must meet other technical standards as specified in the regulations (22 CFR Part 514). This coverage is not intended to replace any insurance a participant may already have. Instead, the intent is to supplement existing coverage and to ensure that a participants basic health is protected in a foreign country.
