- The plan provides a total lifetime coverage amount of $5,000,000.
- Non-US Citizens may avail of this plan wherever in the world they may reside.
- Co-insurance is waived if expenses are incurred within the PPO network and submitted directly to the plan provider.
- Maternity: After 12 mos of continuous coverage plan pays 50% of $100K, then 100% up to LT Max ($250K)
- Newborn care: Under maternity benefit, maximum benefit is of 60 days.
- Mental health: After 12 mos of continuous coverage plan pays $10K/Certificate period, $25K lifetime maximum. Out-patient benefit – maximum benefit 450/day/visit.
- Child wellness: After 12 mos of continuous coverage $50/visit, 3 visits/certificate period.
- Adult wellness: After 12 mos of continuous coverage $250/visit, 3 visits/certificate period.
- Women over 40: $100/certificate period.
