- US$250, $500, $1000, $2500, $5000 and $10,000 deductible. 50% waived within PPO network.
- Co-insurance Inside the U.S. and Canada (Outside PPO network): 80% of the next US$5,000 of eligible expenses after the deductible, then 100% to the overall maximum per period of coverage.
